EVENT INFORMATION.

Impact Sports Academy LLC dba Impact Events must be named as an “Additional Insured” on the certificate. This can be requested from your insurance provider. Site Search is the official hotel provider for Impact Events. • Teams entering a tournament must follow the lodging policy and secure housing through Site Search. • Teams that do not comply may not be scheduled. • A team may opt out by paying the fee shown during registration. • Teams traveling 75 miles or more must reserve tournament lodging through Site Search. • Reservations made through Priceline, Hotels.com, Expedia, or another third party do not satisfy the hotel requirement.

It is required that all teams in the tournament comply with the following rules:Team managers must always have full control of their players and parents. This means “on” and “off” the field. Failure to do so can result in removal from event and/or banishment from future events.In case of disputed play or decision, team managers, only, may consult with the game officials in a respectful manner. The other coaches are expected to remain out of the discussion.This is a NFHS Rule. Here is the lingo and the rule from the rule book. 3-3-1 f6: A coach, player, substitute, attendant, or other bench personnel shall not: commit any unsportsmanlike act to include, but not limited to, any member of the coaching staff who is not the head coach (or designee) in 3-2-4 leaving the vicinity of the dugout or coaching box to dispute a judgment call by an umpire.Managers are to report any unsportsmanlike or derogatory acts by players or spectators to the tournament director. The purpose here is to prevent any serious situation developing that would be harmful to the tournament.Managers, coaches, players, and teams are liable for suspension by not adhering to the tournament rules in effect. This could mean suspension for a game, games, tournament, or a longer duration depending on the act of violation.A player, coach, manager, or sponsor may be suspended for fighting, swearing, any contact with an official on or off the field, obscene gestures.Any intentional destruction of property or abuse of IMPACT EVENTS or IMPACT SPORTS ACADEMY property will be dealt with very sternly. Team managers and sponsors will not be warned other than through this notice. Punishments can be, but are not limited to, fines, ejections, and/or team disbarment from future IMPACT EVENTS and IMPACT SPORTS ACADEMY events.

I, the undersigned player parent/guardian, acknowledge, agree and understand that:Voluntarily and of my own free will, I elect for my child/ward to participate in an Impact Events or Impact Sports Academy, LLC sponsored event.I understand that there are certain risks and hazards involved in participating in baseball/softball that may result in injury or death to my child/ward or other players, including, but not limited to those hazards associated with weather conditions, playing conditions, and equipment.I understand that sliding into a base is dangerous to my child/ward and to other players and may result in serious injury or death.I understand that the very nature of the game of baseball/softball is hazardous and risky, including, but not limited to, the acts of pitching, throwing, fielding, and catching of the ball, the swinging of the bat, running, jumping, stretching, sliding, diving, and collisions with other players and with stationary objects, all of which can cause serious injury or death to my child/ward.Further, I, the undersigned player parent/guardian, agree that in consideration for the right to play as a participant of the youth tournament and in consideration for permission to play on the fields arranged for by Impact Events and Impact Sports Academy:I voluntarily elect to accept and assume all risks of injury incurred or suffered by my child/ward (a) while practicing or playing in the tournament and as a member of the team so designated, (b) while serving in a non- playing capacity as a team member during practice or play by other teams or by other players on my team, and (c) while on or upon the premises of any and all of the fields arranged for by my team or league for practice or competition.As a Parent/Guardian and as an Athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form, I understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury. I agree that my child/ward must be removed from practice/play if a concussion is suspected.I release, discharge and agree not to sue Impact Sports Academy, LLC and/or the field owner or other entity, or their owners, officers, agents, servants, associations, employees, or any person or entity connected with the team, tournament, or field owners for any claim, damages, costs or cause of action which I have, or may in the future have, as a result of injuries or damages sustained or incurred by me from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of any parties hereby participating within the grounds of an Impact Events or Impact Sports Academy, LLC event.

I hereby assume all of the risks of participating in any/all activities associated with Impact Events or Impact Sports Academy, LLC, including by way of example and not limitations, any risks that may arise from negligence or carelessness on the part of the entity being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.To release, waive, hold harmless, discharge, and agree not to sue Impact Events or Impact Sports Academy, LLC, or its members, officers, employees, volunteers, agents, students, and staff (hereinafter referred to as “releases”) from any and all liability, claims, actions, demands, expenses, attorney fees, breach of contract actions, breach of statutory duty, or other duty of care, warranty, strict or other liability actions, and causes of action whatsoever, that I might now have or may acquire in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while training for, traveling to or from, or participating in an Impact Events related activities.The participants are physically sound. The undersigned represents that participants are mentally and physically sound and suffering from no condition or impairment that would prevent the participant’s safe participation in the instruction and physical activities offered by the Academy and all Academy Activities. The undersigned agrees to keep the Academy personnel informed of changes to the Participant’s physical condition or changes in the Participant’s ability to perform the activities associated with the Participant’s training, the instruction and the Academy activities.I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns.

No refunds will be issued after payment. If a team cancels at least two weeks before the event, Impact Events will hold the payment as a credit toward a future event. If an event is canceled or shortened because of weather: • If Impact Events cancels the event, the team will receive a 100% refund. • If none of the guaranteed games are played, the team will receive a 90% credit toward a future Impact Events registration. • If 25% of the guaranteed games are played, the team will receive a 50% credit. • If 33% of the guaranteed games are played, the team will receive a 40% credit. • If two or more games are completed, the event is considered a full tournament. TEAM WITHDRAWAL • Teams that withdraw after an event is sold out will not receive a refund or credit. • Teams that withdraw within 90 days of an event will not receive a refund or credit.
